Leave of Absence

If you are taking a break from practicing as a midwife, it’s beneficial to change your membership status to Inactive: your fees with both the AOM and CMO will be reduced; and costs to the health-care system are reduced, too, as you do not need liability insurance coverage on leave.

A member who is Inactive is still a registered, voting member of the AOM and

  • can still vote at the Annual General Meeting;
  • will receive updates about contracts and negotiations;
  • will have access to AOM On Call, our confidential member support line; and
  • will remain enrolled in the AOM Benefits Trust program (i.e., health, dental, disability, life insurance etc.) for up to 12 months contingent upon approval from the insurer and payment of monthly premiums.

Going on Leave Checklist

Returning from Leave Checklist

(4-6 weeks before returning)
  • Discuss your leave plans with your practice. Make sure you talk about
    • the timing of your leave (start and end dates);
    • compensation (i.e., which clients are billed to you after your leave begins? How will you handle compensation for clients who were partially in your care? etc.); and
    • liability insurance.
  • Change your status with the CMO.
  • Change your status with the AOM, and return this form by email, mail or fax (416-425-6905)
  • Contact AOM Benefits Trust to discuss implications of a leave on your participation in the Benefits Program.
  • Check with your hospital about privileges.
  • Contact your TPA (if necessary) to allow for billings once your CMO class becomes Inactive.
  • Discuss your return plans with your practice, including start date and insurance coverage needs.
  • Contact your hospital to reactivate privileges, if you suspended them.
  • Change your status with the CMO.
  • Change your status with the AOM, and return this form by email, mail or fax (416-425-6905)
  • Reapply for liability insurance coverage (confirmation from TPA may be necessary) and return this form to the AOM by email, mail or fax (416-425-6905)
  • Contact AOM Benefits Trust to re-enroll in the benefits program within 31 days of returning to practice.
