BORN Data Entry Resources

Good quality midwifery research, and advancements in midwifery funding, policy, and practice all begin with good quality data entry. However, entering data into the BORN Information System (BIS) sometimes poses challenges for midwives. The AOM continues to work collaboratively with BORN to recommend and support updates to the BIS that reflect the unique aspects of midwifery care.

To improve the ease of data entry for midwives, the AOM, in collaboration with the BORN Midwifery Advisory Committee and the AOM-BORN Knowledge Translation Task Force, has created the following data entry resources.

Labour & Birth BORN Data Templates Opens in a new tab

The Labour & Birth BORN Data Templates are intended to assist midwives in tracking important clinical information during labour and birth that is captured in the BIS but is not captured in existing client records (i.e. antenatal records or midwifery labour records and hospital labour and birth summaries). Midwives may use these tools to track client information during the labour/birth or in the early postpartum period to facilitate entering data into the BIS at or prior to client discharge from midwifery care.

These forms should be used solely for data entry purposes and should be destroyed once BORN data entry is complete. Midwives are responsible for protecting personal health information (PHI) (login required) against unauthorized use or disclosure, loss or theft; notifying a client if their information is accessed inappropriately, lost or stolen; ensuring that records are accurate and complete; and storing, transferring and disposing of records in a secure manner.

MPGs are welcome to use and adjust these tools to suit their needs.

Two versions of the Labour & Birth BORN Data Template exist, a “Checkbox Version” and a "Open Field Version”:

Checkbox Version (PDF, 335.46 KB)

The checkbox version of this tool offers midwives the full array of checkbox options for each field. This template is a 2-page document that can be printed on a single double-sided page, with room for additional notes. This version may appeal to midwives who prefer the options to be identical to the language used in the BIS and who prefer checkboxes to capture data.

Open Field Version (PDF, 280.82 KB)

The open field version of this tool offers midwives the flexibility of open fields of response, as opposed to the full array of check boxes as they appear in the BIS. This template is a 2-page document that can be printed on a single double-sided page, with room for additional notes. This version may appeal to midwives who prefer to enter responses narratively.
 


EMRs ONLY - Labour & Birth BORN Data Tracking Prompt (DOCX, 20.2 KB) Opens in a new tab

Labour & Birth BORN Data Tracking Prompt is intended specifically for midwives who use an Electronic Medical Record (EMR) to input client information at their midwifery practice groups. The text in this Microsoft Word document can be copy and pasted into an EMR or adapted to best suit your practice as a prompt to capture data elements from the labour, birth and early postpartum period which not captured in other client records. 

This document contains the same information and is intended for the same purpose as the above Labour & Birth BORN Data Templates. Like the above forms, this form should be used solely for data entry purposes, it does not comprise part of the clinical chart and should be destroyed once BORN data entry is complete.


BORN Data Entry Frequently Asked Questions (FAQ) Opens in a new tab

The BORN Data Entry FAQ provides direction and guidance on appropriate data entry based on real concerns raised by midwives. The responses in this FAQ were developed in collaboration with the AOM's BORN Knowledge Translation Task Force and staff at BORN Ontario and are updated as needed.



Consultations and Transfers of Care Frequently Asked Questions (FAQ) Opens in a new tab

Consultations and transfers of care are a unique aspect of data entry for midwives. This FAQ compliments the infographic by providing further guidance on accurate data entry for consultations and transfers of care in the BIS. The responses in this FAQ were developed in collaboration with the AOM's BORN Knowledge Translation Task Force and staff at BORN Ontario and are updated as needed.
